# Missing, Error, Update Needed for Payers, Forms, & Service Codes

When you need to request a missing payer, form, or service code, or report an error with any of these, use the form linked in the product.

The form covers all three request types in one place:

- Missing payer, form, or service code
- Error with an existing payer, form, or service code
- Update needed to a payer, form, or service code

##### Where to find it

The link to the form lives in the following three places:

1. **Submission prep → Edit form** — the "Report form errors" link at the bottom of the form preview

2. **Submission prep → Choose form** — the "Report missing form" link below the form options

3. **Patient insurance panel** — the "Contact SamaCare support" link under the insurance company field

##### How it works

1. Clicking the link opens the request form in a new tab, where you can complete and submit it.
2. In the form, first select the topic that best matches your request:
   
     - **Forms**
     - **Payers**
     - **Service Codes**
3. Then select the request type:
   
     - **Error** 
     - **Missing**
     - **Update**
4. Add any additional details that will help our team review and resolve your request.
5. Click **Submit**.

💡In Step 3, depending on the request type, you may be prompted to upload a screenshot of the error you are experiencing.

Once submitted, your request is routed to our internal team. They'll follow up via email once it's completed, or sooner if they need any additional information from you.
